Following are frequently asked questions from clinicians and other providers in the LifeSolutions network.
| How do I go about joining LifeSolutionsnetwork? |
You may download a LifeSolutions Application (LifeSolutions Affiliate Clinician Profile) by clicking on the link at the left. Complete the application, attach the required documents, and mail to the address indicated on the application. Your application will be forwarded to the LifeSolutions Credentialing Department. The process takes approximately 2-3 weeks. |
| How do I check the status of my application? |
Call LifeSolutions at 1-800-647-3327. |
| How much paperwork is required? |
LifeSolutions strives to keep paperwork to a minimum. You will be asked to submit a Case Opening, SPS-6, Case Closing, and Reimbursement form at the completion of each care episode. |
| What are your fees and how do you determine them? |
LifeSolutions fees are based on market rate conditions as determined by our surveys and comments from providers, purchasers, and members. Session reimbursement fees will be specified in your provider contract. |
| Whom should I call if If I have a question regarding a client case? |
You should call LifeSolutions National Call Center at 1-800-647-3327 and speak to an experienced Care Manager who will staff your case and answer your questions. We also have a confidential after-hours voice mailbox where you can leave a message and receive a call back the next business day. |
| How many members do you cover (especially in my area)? |
LifeSolutions is constantly growing its business nationally. If you have received a phone call or letter from us, there is a good chance that LifeSolutions is developing the network in your area, and that we expect new business to come on line. | How quickly will I be reimbursed? |
